To access the assignment submission form, you need to login to the Delta Investigative Services website.  The login box is located in the lower left hand corner of the website, as seen below.

Once you're logged in, you'll have access to additional menu items in the upper left hand menu.

Clicking on the Submit an Assignment selection will take you to the assignment selection page where you will have the choice to submit an insurance assignment or corporate assignment.

From here, select from the 3 options for submission (form, download or e-convo.)
